Abstract: Women with epilepsy (WWE) account for 0.3–0.7 % of all pregnancies, yet their care remains one of obstetrics’ most complex balancing acts. Clinicians must juggle maternal seizure control, teratogenic risks of anti-seizure medications (ASMs), rapidly changing drug pharmacokinetics, and heightened obstetric morbidity—all while supporting reproductive autonomy.
