Abstract:
Background: Discharge planning is a process where patients’ needs are identified , and a plan formed for a smooth transfer from one environment to another. A discharge plan means puts down on paper (writing ) the end-goals of the care plan, which ultimately aims to empower the patient to make decisions and be resilient, to maximize his potential to live independently (Driscoll , 2000) Nurses' knowledge and inadequate information are the most common barrier for putting down patient discharge planning .
